Rehabilitace představuje neoddělitelnou součást komplexní léčby nemocných po infarktu myokardu. Jedním z hlavních cílů rehabilitace je umožnit nemocným návrat k pracovním a rekreačním aktivitám v maximálním rozsahu, jak jen je to možné. Základem pohybové aktivity je aerobní trénink.

Mechanizmy, kterými fyzická aktivita a zdatnost snižují mortalitu, nejsou zcela jasné. Jsou samozřejmě spojené s redukcí rizikových faktorů. Efekt fyzické zátěže však nemůže být vysvětlován pouze redukcí rizikových faktorů, protože se uplatňuje jako nezávislý faktor snížení mortality.

Rehabilitační proces se zpravidla dělí na čtyři fáze. Nemocniční rehabilitaci, časnou posthospitalizační rehabilitaci, období stabilizace a fázi udržovací. Časná posthospitalizační fáze se pokládá za rozhodující pro navození potřebných změn životního stylu a dodržování zásad sekundární prevence.
