Bolesti dolních zad degenerativního původu představují díky četnosti výskytu (až 60% populace) výrazný socioekonomický problém. Před započetím jejich léčby je nutno vyloučit diferenciálně diagnosticky především jiná závažná onemocnění - nádory, zánětlivé afekce, přenesené bolesti viscerální. Rovněž je nutno posoudit možný psychosociální vliv podílející se na problému. Léčba je vždy na prvním místě konzervativní, při jejím selhání je možno využít semiinvazivní analgetické techniky (epidurální injekce, katetry, intradiskální elektrotermální terapii). U progredujících stavů, rezistentních na výše uvedené způsoby terapie, připadá v úvahu léčba chirurgická. V posledních deseti letech jsou to především operace s cílem rekonstrukce disku a pohybového segmentu, tzv. -non-fusion- techniky. Přes velký pokrok léčebných metod však považujeme stále za zásadní prevenci.
