Akutní pankreatitida

Akutní pankreatitida je primárně neinfekční zánětlivé onemocnění slinivky břišní. Přes nesporné pokroky v diagnostice a terapii zůstává mortalita těžké formy vysoká. Patofyziologie onemocnění není zcela objasněna. Pankreatitidy jsou převážně biliární etiologie nebo vyvolané abúzem alkoholu. Diagnóza onemocnění je postavena na klinickém obraze, zobrazovacích metodách (zejména ultrasonografie a počítačová tomografie, event. ERCP) a laboratorních vyšetřeních, ze kterých jsou v rutinní klinické praxi nejčastěji používány – CRP, amyláza, lipáza a prokalcitonin. Používání skórovacích systémů může usnadnit časnou identifikaci rizikových pacientů a umožnit tak okamžité zahájení intenzívní léčby. Kauzální terapie akutní pankreatitidy není dosud známa a současná léčba je tak postavena na komplexní multidisciplinární intenzivní péči se zaměřením na dostatečný obrat tekutin. Profylaktické podávání antibiotik nemá jednoznačnou oporu medicíny založené na důkazech. Chirurgická léčba je rezervována pro pacienty s prokázanou infikovanou nekrózou pankreatu, zejména je-li spojená s rozvojem sepse a multiorgánové dysfunkce. U biliární pankreatitidy je indikována léčba endoskopická.
